There's a couple of Doom 2 levels where we cannot get 100% secrets because John Romero placed the secret in a teleporter sector (and the player would never be able to enter the sector without being teleported).
15: Industrial Zone
27: Monster Condo
Any others?
There's one level I can get 100% items but only 92% kills is 29: The Living End. I just cannot find any more monsters. Is this a bug or normal? Does anyone else get this?

I don't think either Map 15 or 27 has unreachable secrets.
In Map 15, are you perhaps thinking of the teleporter that takes you to the "Super Secret Level" Map30? You need to get there by first pressing a switch on a pillar in the lava, and you can only get to that pillar by stepping off (without running) from the island in the middle.
In Map 27, please tell me which teleporter you're talking about, because I can't remember any such area.
I remember getting 100% kills & 100% secrets on all levels. So I think that maybe you're missing something in Map29.

Hmmm. I am thinking you are correct andwan0. Here is what I am discovering in the editor.
In map27, sector 93, a teleport pad in secret area with health potions, is secret, but has teleport line in front of it, so player cannot reach it.
In map15, sector 148, one in front of teleport to rocket launcher tower, is set as secret, but it is 16 units wide with teleport line behind it. I am noticing teleport lines trigger when thing is halfway through, so maybe since the player is 32 units wide, the player cannot get far enough into sector to count as being in it if this property is also true for counting which sector a thing is in?
I am completing map29 with defeating all enemies. The ones I am thinking easiest to miss are 5 imps around switch with arch-vile in front of it and also of 6 cacodemons near soulsphere sometimes some float away into teleporters to start area.

The MaxSec (http://www.classicdoom.com/maxsec.htm) guide gives the normal maximum Secrets-found scores for all the levels of the game, and explains the typical sub-100% scores.

Hey guys, just to let you know I found my 3 lost missing monsters. It was the 3 imps up in the hard-to-see outside area where the archville protects the switch that opens the pink (tougher) hell knight who protects the teleporter to the final middle mountain (map 29).
Yes, they were hidden away but I found them. They don't see me and I had to shoot thin air and then they responded.

Yeah I used to have problems with those three too. Then again I was a whole 7 or 8 years old at the time. Since then I'm gettin %100 everything.. Also some people think they got everything but those sneaky Arch-Villes brought a few back, so just a reminder make sure whats dead, stays dead.

Yeah, I just tried on Map 15, that secret is impossible to get on GZDoom, Doomsday, Doom Legacy and Chocolate Doom. I haven't tried it on Doom95 yet, but I doubt you can get it unless you use noclip. I don't see anything wrong with Map 27 though, the teleporter works fine for me.
On the Living End for all kills, are you playing it on UV?
