I'm pretty sure it was DEU, you can still get it too. Just hunt through the old archives, I'm pretty sure I've seen it there.

I don't like DEU, too primitive for me. I prefer sector based map editors, not vertex based

It wasn't DEU. That was a third party program and I can't even remember who wrote it. It was slow to use and buggy but it wasn't bad compared to the alternatives of the day.
No idea what Id used though.

id used a program on their development machines. I think it runs under some form of UNIX. It's on the Rome.ro forum (http://rome.ro/cgi-bin/ultimatebb.cgi) if you really want to find the answer.
