Well sign language is not that hard to learn, really. Sure, it's hard to know the whole language and the grammar, but, knowing just enough sign language to communicate with people is easy. YOU know quite a number of signs, you just don't realize you know it! Trust me on this.

As for the cacodemon, I don't think his brain is full of air. I think cacodemons are meaty and their brains are quite small, like a peanut or the size of your fist, at most.
